Evanston community health center offers: Medical, behavioral health, pharmacy and health coverage assistance. We accept most insurance plans including: Medicare, medicaid, chip, and pcn. We also offer a sliding fee scale for qualified patients. All patients will be seen regardless of ability to pay.
